Reservoir Hose to Tee - 3 versions? (Updated)

Hey guys,

Fighting another coolant leak on our 2012 LTZ hatchback M/T (1.4). It looks like the original heater/coolant hoses are starting to swell and leaking at the tee behind the passenger side of the engine. I've found the heater inlet and outlet hose p/n's (96958216 & 96958209 in case it helps someone), but can't verify the long hose that runs in front of the engine along the radiator from the reservoir back to the tee.

RockAuto lists two different p/n's (one 5x the cost of the other), and ours is something different, entirely: 95164047....this doesn't cross on any of the parts sites. I'm assuming it was superseded, but by what, I don't know. Anyone familiar with the differences?

I also would really like to know the answer to this one. I have a coolant leak in the same place. I can see it dripping where all 3 hoses come together.

Here's an update now that parts are in.... Initially we had a leak on the passenger side, and this turned out to be a bad o-ring/gasket on the t-stat housing. Easy/cheap fix though a battle getting at the lower bolts. A week later we had a leak from the driver's side. This turned out to be a failing (stress cracks) reservoir; another easy/cheap fix. Another week goes by...coolant leaking from the passenger side again. This time accompanied by a shredded serpentine belt.

Interesting failure...the belt was still in one piece, enough to drive all accessories and not trigger any warning lights, yet now it was only 1/4" wide! Replacement did involve jacking up the engine and pulling a motor mount, but wasn't awful.

Now for the leak..... when the belt shredded, it sliced the plastic tee for the cooler lines. Just nicked it enough to cause the smallest of leaks, but I could tell all the old hoses were swollen and after 100k, should be replaced. I did the 3 from the tee, a small bypass on the front drivers side, and the remaining heater hose at the rear. I left the large ones in place for now. Take a look at pic 3 to see the swelling (sorry about the perspective).

The new GM replacements had different part numbers than ours, but fit was pretty much identical. The plastic tee was marked 95-368300. There's no GM replacement that I could find, but Dorman makes one that's popular. Problem is, the reviews on it are awful, and failure is pretty much guaranteed. Instead, I was able to find a 3/4" brass "hydrant tee" at the hardware store of roughly the same dimensions, and bonus, it was 2/3 the price of the plastic one.
